This notice explains how health information may be used and disclosed for treatment, payment, healthcare operations, and legally required purposes.

Our legal duty

Applicable federal and state law requires covered healthcare operations to maintain the privacy of health information and provide notice of privacy practices, legal duties, and patient rights.

Privacy practices may change when permitted by law, and updated notices apply to health information maintained by the organization.

Uses and disclosures

Health information may be used or disclosed for treatment, payment, and healthcare operations, including physician review, payment processing, quality activities, credentialing, and operational support.

Health information may also be disclosed when required by law, for FDA-related reporting, in abuse or neglect situations, for national security or law-enforcement circumstances, or with your written authorization.

Patient rights and complaints

You may request access, amendment, accounting of disclosures, restrictions, alternative communications, or a written copy of the notice, subject to legal limitations.

You may file a privacy complaint with B12Rx or the U.S. Department of Health and Human Services. Retaliation for filing a complaint is not permitted.
